Special Event Equipment Use Application

  1. DISCLAIMER
    The Special Event Equipment Use Application serves as a REQUEST ONLY. Submission of an application DOES NOT GUARANTEE use of the equipment. All applications are reviewed by the Parks & Recreation Department on a first come, first served basis. Once your application has been reviewed by staff, you will be contacted directly to verify confirmation or denial of your application. All applications must be submitted at least 30 days prior to your event. For questions, please contact Stuart Brown, Parks & Recreation Director, at (760) 965-3696 during business hours.
  2. ALL APPLICATIONS FOR USE OF MEASURE R EQUIPMENT SHALL PROVIDE THE FOLLOWING INFORMATION:

  5. REQUESTED EQUIPMENT
    Please enter the quantity of each equipment item you would like to select for your reservation. Available quantities are listed underneath each item. Please input a "0" for any equipment items you do not need. Available quantities are the totals of our inventory, they are not a reflection of what is available for rental. For example, another event may have already reserved a portion of the inventory. If your requested amount exceeds what is available you will be contacted by the Parks & Recreation Department.

  16. Additional Requirements
    * Requires special training with A Plus Certified. ** Must be returned with a tank of gas.
  17. Equipment Use Deposit*
    All applications must submit a refundable $250 equipment use deposit to the Parks & Recreation Department. Deposits are accepted in the form of cash or check (made out to TOML). If you are also applying for a Special Event Permit, your deposit check must be separate from your special event permit fee check. If it is determined that there was no loss or damage to the equipment you rented once it is returned, you may pick up your deposit at the Town offices or request to have your check shredded.
  18. EQUIPMENT PICK-UP & DROP-OFF PROCEDURE

  20. The equipment use agreement (once approved) shall begin on the date specified, and expire on the specified date in the application. At the end of this time period, if the applicant wishes to keep the equipment for additional time, the event producer needs to contact the Parks & Recreation Department for approval prior to such activity.
  21. In the event that problems arise due to the conduct of this activity, the Parks & Recreation Department Director/Manager reserves the right to review this permit and attach additional conditions as deemed appropriate, or to revoke all privileges granted by this permit.
  22. It shall be the applicant's responsibility to know and comply with all County, State, Federal, and local laws.
  23. It is understood that the activity mentioned above and on all attachments involves an element of risk associated with the activity, whether known or unknown, and whether known or unknown to the persons and entity named above.
  24. In consideration of the acceptance of the rented equipment, the applicant hereby indemnifies, holds harmless, waives, releases, and discharges in advance, and shall require and cause all participants and volunteers using the rental equipment to so indemnify, hold harmless, waive, release, and discharge in advance, the Town of Mammoth Lakes, including its officers, agents, servants, volunteers and employees, from any and all claims, causes of action, damages, penalties and liability, including, but not limited to, those arising out of death, bodily injury, personal injury, and property damage occurring as a result of said activity, notwithstanding any negligence or carelessness on the part of the persons or entity named above and even if such persons and entity would otherwise be liable.
